Evaluation Forms Extension - Working with Custom Evaluation Forms

Note - This feature is only available for those who have purchased the Evaluation Forms extension.  If you are interested, please get in touch with Lawren Finley.

With custom evaluation forms, you can include the legacy 5-star and comment system for courses and instructors, as well as create your own questions in the form of multiple-choice or free-text fields.

Steps

To create a new evaluation form, from the +New menu, choose Evaluation Form, OR from the More menu, choose Evaluation Forms, and then click the + New Evaluation button.

Profile

Title - Give your evaluation a Title, e.g., Security Awareness Feedback

Description - Consider adding a description that explains the purpose of this evaluation.

Moderators

Send Alerts - Enable this if you want UniversitySite to notify any specified moderators when feedback for this evaluation is submitted, but don't forget to specify at least one moderator below.

Moderators - Add any people you want to be able to moderate feedback comments submitted for this evaluation. You can specify multiple. Moderators can not only review and publish feedback, but they can also edit and delete submitted feedback if appropriate. If desired, specify moderators by department so that if a course is assigned to a department and feedback is submitted, that department-specific moderator will have the right to moderate that feedback.

Questions

Collect Course Ratings - Enable this option to collect the star and comment on COURSE ratings in your evaluation.

Collect Instructor Ratings - Enable this option to collect star and comment INSTRUCTOR ratings in this evaluation.

Add Question - Use this to add custom multiple-choice or free-text questions.

Note: For each question, you can decide whether the answer is required AND whether to allow comments.

After adding your questions as desired, be sure you click the blue Save Evaluation button.

If appropriate, after saving, you can click the blue text to make this evaluation the default evaluation which will be selected when you create a new course.

Yes/No Type Question

Here is an example of a Yes/No question that uses the Multiple Choice type question and only requires two options.  If you are creating a question that will have more than two response options, use the Add Response Option button to add those options.

In this example, I used a multi-line text field type to allow them plenty of room to enter their responses.  If I was looking for a short response I could have used single line text instead.

When you Save your evaluation, you should see something like the below:

Editing Questions

You can edit questions at any time. However, if the evaluation has already been submitted by a user, you will see a warning:

You can click and unlock the question and then edit. The edit will not affect evaluations that have already been submitted. They will stay the same.

You can change the question type by first expanding the question:

You will then be able to click on the dropdown for the question type:

Assigning This Evaluation to Courses

Now that you have created your evaluation, it can be specified when editing a single course from the evaluation section, OR you can assign it to multiple courses by clicking 

Select the courses you want to assign this evaluation to and then click the + Add Evaluation to selected Courses button.

Then type the requested text and click Add to confirm this bulk action.

Reviewing Submissions

To review submissions:

  1. Go to the More menu, Evaluation Forms, and click the form with the feedback you want to review.

  2. Click the Submissions tab. Please note that you have the ability to apply a variety of filters, including by Course, by Instructors, and by feedback Status. 

  3. To see individual feedback, click Show Details.

  4. To review multiple submissions, use the Download feature or from Reports | Learners | view the Evaluation Submissions - Feedback report.
