Evaluation Forms Extension

Background

UniversitySite provides the ability to collect a 5-star rating on a course and each instructor. The Evaluation Form Extension adds the ability to create multiple evaluation forms and to add questions to evaluation forms. With it, you can create course-specific evaluation forms or one evaluation form for Instructor-Led Courses and another for On-Demand Courses. In addition, you can collect information about the quality of the food, the facility, technology, etc. You may even want to ask questions about specific topics that were covered such as, What are the risks of opening an email attachment?

Do I need the Extension?

Not necessarily. Without the extension you can:

Collect a 5-star rating on a course
Collect a 5-star rating on each instructor
Require comments on low ratings
Opt-out of collecting instructor feedback
Decide on a course by course basis if feedback will be collected

What does the extension add?

With the extension, you can:

Design a firm-specific evaluation forms with your questions
Design an evaluation form for Live Events with your questions
Design an evaluation form for On-Demand courses with your questions
Design course-specific evaluation forms with your questions
Design department-specific evaluation forms with your questions

Adding Questions

Below is a screenshot of the Evaluation Form Editor. Without the extension, you cannot add questions. With the extension, you can add questions.
